Service Description

Creating a Positive Behavior Support Plan (PBSP) is a collaborative and person-centered process. It begins by building a team that includes the individual and important people in their life, such as family, friends, coworkers, and providers. These initial meetings focus on getting to know the person, understanding their perspective, and gathering information about their history and past experiences. Direct observations are conducted in multiple settings to identify the antecedents, behavior, and consequences related to the target behavior with careful attention paid to any situations that might pose a risk to living a high quality life. This information is combined with a review of existing records and ongoing data collection to provide a comprehensive understanding of the behavior's function. The PBSP is built upon the person's strengths and focuses on their needs, preferences, and goals. A Functional Behavior Assessment (FBA) is conducted to understand why the person is engaging in the behavior, and strategies are developed to address the function and teach replacement behaviors to meet the same need while building a safe and inclusive environment. The family and support team are trained to the plan which includes prevention strategies, skill development, environmental modifications, and relationship building. Regular reviews of the data allow the team to discuss challenges and make necessary adjustments to the plan, ensuring it remains a living document that adapts to the person's evolving needs.
